To complete the application process...

1. Visit www.caddosmart.com or click the "Click Here to Apply" button on this page

2. Create an account, upload documents, and complete the application

3. If you are unable to upload documents, need assistance, or have questions/concerns, please contact PreK-2 Counselor, Mrs. Olivia Caldwell-Evans.

To register please provide the following documents:

• Proof of Income (2 consecutive pay stubs, or SNAP benefits with child’s name)

• Proof of Residence (current utility bill)

• Birth Certificate

• Parent Driver’s License

• Current Immunization Record
